For the treatment of advanced or metastatic breast cancer, the recommended dose of Lapatinib is 1,250 mg (equivalent to 5 tablets) taken orally once daily continuously on Days 1-21, in combination with capecitabine 2,000 mg/m2/day taken orally in 2 doses about 12 hours apart on Days 1-14, in a repeating 21-day cycle.
For hormone receptor-positive, HER2-positive metastatic breast cancer, the recommended dose of Lapatinib is 1,500 mg (equivalent to 6 tablets) taken orally once daily continuously, in combination with letrozole at a dose of 2.5 mg taken orally once daily. Lapatinib should be taken at least one hour before or one hour after a meal, while capecitabine should be taken with food or within 30 minutes after food. It is important to take Lapatinib as a single daily dose and not divide the dose.
The dose of Lapatinib should be adjusted in case of cardiac and other toxicities, severe hepatic impairment, diarrhea, and CYP3A4 drug interactions.
