Obeticholic Acid Dosage

After the initial three months, patients who have not achieved an adequate reduction in ALP and/or total bilirubin and are tolerating Obeticholic may have their dosage titrated. Non-cirrhotic or compensated Child-Pugh Class A patients may be prescribed 10 mg once daily, while Child-Pugh Class B or C patients or those with a prior decompensation event may begin with 5 mg twice weekly (at least three days apart) and titrate to 10 mg twice weekly based on response and tolerability.

The maximum dosage of Obeticholic is 10 mg once daily for non-cirrhotic or compensated Child-Pugh Class A patients and 10 mg twice weekly (at least three days apart) for Child-Pugh Class B or C patients or those with a prior decompensation event. Prior to any dosage adjustment, it is important to recalculate the Child-Pugh classification. Decompensation events include gastroesophageal variceal bleeding, new or worsening jaundice, and spontaneous bacterial peritonitis.
